Password is prohibited?

Hi, good afternoon!

I tried to use a password (conditional value for a text field) in order to show question only if the password is correct. I would like to use that mechanics in order to avoid unwanted and randon responses.

Twice I assigned that tool for the form, and twice it let all the form disabled.

Could you help me to have the form back?
