Hello... Where is the setup& embded tab...?

Our apologies. Somehow no link was included in my colleague's reply. However, the "Setup & Embed" tab can be found near the top of the form builder. The "Embed Form" button on that tab is highlighted below:

The Adding Feedback Button to Blogger guide has been updated accordingly.

Oops! My mistake. It was my browser that for some reason didn't load the image in Pinoytech's reply.
