Can you create accounts to password protect a form?

    I am working for a optical shop interested in a HIPAA compliant form system.  Their current in house form requires a user name and password to send forms and amends the user's info to the form.  Is there any way to duplicate this with Jotform?

    Hello James,

    Unfortunately it is not possible to create regular authentication form using JotForm. You cannot validate input data with any external source or previous submission data of form.

    Please be noted that you can add "Conditions" in your form to check the input value of a text field and accordingly show / hide rest of the form. You may like to take a look at the following thread where I have explained this workaround in detail:
