how do I change the form tab

I'm not sure if I understand properly your question. But if you're referring to the form tab titles, you can change the title in the form tabs by editing your form and then selecting the form tabs element in the form. This will get the element highlighted and you will notice on the right part of the form there are additional options that will appear -- the wizard icon, properties icon, and the delete icon. 

Next, click the wizard icon. A window will appear and then on the Tab Titles, rename them accordingly. Please take note that for every new line you make, that is considered as a new tab.

Please take note that the tabs will only appear if you have the page break added in your form.
