I have a website service with wix.com and used jot form for our application service. It has been working for three years and all of a sudden stopped working and im not too sure why. Any help would be greatly appreciated. Thank you.

I think you mean when the form is submitted? That is because your Thank You URL is incorrect. You have it set to redirect to a text value, not a URL.
I've fixed this for you by using our default thank you page:
Also, we are now securing forms by default. You may consider re-embedding the form. This time, I would suggest using our iframe method: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
