uploading an existing questionnaire form

Help! I cannot for the life of me figure out how to upload an existing form

Can you please elaborate a bit more on how and where you wish to upload the form? I notice that it is a simple form with one single field (upload field). Are you trying to embed the form into your site. If so we recommend using the iFrame script.

If this is not the case, please feel free to correct me.

I would like to upload a questionnaire from my documents on my computer. How do I upload the whole form and start plugging in the text fields

I'm sorry but not fully understanding you. IF I am correct, you have a form which you wish to upload into Jotform. If this is the case, please let us know the type for form you have. If it is a fillable PDF, you may use the following link:

http://www.jotform.com/adobe-formscentral-alternative/

This will enable you to import the PDF form into your account.

If not you would require to create the form from scratch. If you like, we may assist you creating the form.
