Form is disabled

I have just signed up and started to create a form and is wont let me display it

Says is not found - disabled

Your account was automatically suspended because of the form that asks for a certain type of card. I guess the filter thought your account was asking for credit cards. But upon review it seems to be asking for a different type of card. So I went ahead and reactivated your form. You should be able to access your form again.
