My form is not working. It says that the form has been disabled.

    My form is not working. It says that the form has been disabled. This is the second time this has happened. Once before it went live and now again.

    Your form is disabled again for same reason as last time. Inside of the form http://www.jotform.us/form/61628079045156  you have fields that collect credit card information and that is against JotForm Terms of use.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    I have removed those fields from your form. Please do not create forms that are against our terms of use, or your account will be suspended again. 

    I would also suggest you to change title of that form. 

    If you want to have payment fields inside of your form you have to use one of JotForm payment tools.
