Confused RE "file upload" that appears in the basic fields tab. In reading the forum on uploading documents from a form there are numerous reference to a placement of a widget.? Is a widget separate from the "file up load' required?
If you are looking for normal file upload feature that allows to add files to the form submissions, the basic file upload is sufficient. I have checked your JotForm and see that the file upload field is working fine.
We do have some widgets that allow you to upload the files that you may want to use on your JotForm. These widgets are alternative to the basic file upload with some added features.
However, they are not required to be added if you want to use basic file upload field.
