I have tried several times to get the test emails to send to email@example.com, but I can't get them there. I've checked the spam folder, and they aren't there. I've put a different email address and I'm not getting it there either. I'm not being notified when someone fills out our form, and we desperately need those emails.

I tested your form and I haven't experienced any issues with receiving email notifications. However, I can see many failed attempts of sending emails to your address.
Could you please try with some free email address(Gmail for example) and let us know if it works?
If you can receive the email with the free email address you should try contacting your mail service provider and check if there is some issue with them.
