Date information is not showing up in the form email

Would appreciate help in where I'm going wrong.

This form https://eu.jotform.com//?formID=61253228887362 prepopulates this form https://form.jotform.co/60946887625876, the forms are being filled in, but my email notification does not have the date information etc in it.

You may consider re-adding the fields in the body of the email. Just click the Form Fields option from the toolbar:

You can also re-create the notification email.
