My form looks distorted when I preview it. However, it looks fine on the screen when I am building it

1) My form has the questions distorted.  Can you please help.  Also

2)  I have multiple choice survey questions.  On a few of the questions the last options is Other and/or allows a free form answer.  Is there a way to put the text box right under the option

To fix the issue, please seat the Label Align of the fields on the form to Top. Currently they were set to Auto
