why is my form disabled?

    I'm guessing because of what it's asking for. I use jotform forms all throughout my business if you glance around www.abrighteravenue.org . I just made a new jotform log in for this particular form because i don't want my other administrators to have access to this info.  My original account has been in good standing for a long time (username abrighteravenue)   Can someone enable this form or do I need to do something different?  Thanks!

    Hello ABApayments,

    Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Credit Card Authorization" has fields asking for customer's credit card number which makes it suspected of being used for phishing.

    Please be noted that you should not ask credit card information from your users. If you need to process payments, please use one of our integrated payment tools. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities. It's just a matter of time and our anti-phishing filter will disable your form and suspend your account.

    I have deleted the credit card related fields and activated your account again. Kindly refrain from creating form with email/password, login/password or credit card details.
