I set up the telephone numbers correctly with input mask; when a registrant input her telephone #, it was in the correct formate, (999) 999-9999. Now something happened, even with input mask enabled, the telephone# shows up like 9999999999.
What happened? Please help. Thank you.

I have tested your form and I was able to replicate the issue, the mask is not being applied, but this happens only when the widget "Preview Before Submit" is added to the form, I can see you're using this widget, removed it from your form and then the input mask started to work.
I will report this issue to our second level and our developers will take a look to the issue, we will let you know when this is resolved.
