Yes, this seems to happen when some of JotForm's resources responsible for the styling were incorrectly loaded. Or it might also be related to a CDN issue. In my case, the problem is not visible.
You should be able to fix this by simply logging out and clearing your browser's cache. After that, try opening the JotForm's website again and see if the problem persists. If it's a CDN issue, it's usually temporary and should fix itself in no time.
