Why are my Forms Disabled after I added them to my Weebly Website?

Hi, I added JotForm as an app to my Weebly site and after building the form, it will not display.  I get a message saying the form is disabled.  I am able to access the form I built in JotForm.  I tried embedding HTML code into the site as well as dragging-and-dropping the app onto my page as I have done with other apps.

The reason why your account was disabled is because one of your forms got your account suspended for violating our terms of usage because it is asking for usernames and passwords and that is not allowed since we consider it phishing for sensitive information.
