My form has been disabled and my account was suspended

    Dear Admin,

     

    This form has been disabled and the account associated with it was also suspended.

    Please advise what went wrong?

    Many of your forms are asking for credit card information using our plain text fields. Please be informed that we don't allow this type of form. It's against our Terms of Use (http://www.jotform.com/terms/). 

    I have reactivated your account, but I have deleted the fields from the forms. Please refrain from adding them again. 

    If you'd like to collect a payment at a later date, you can integrate your form without payment tools and enable authorization:
