I have the page open to integrate me form to salesforce, but it won't authenticate even though I went onto sales force and reset my password and security token to make sure I was providing the correct one. I was also wondering if I sign up for your services and I only need to use the form for 3 months of the year can I pay for three months and then stop paying and start again next year?

I cloned your form and integrated it with my salesforce account. It seems that I am able to integrate the form to salesforce successfully.
As for your other concern, you can choose to subscribe to jotform's monthly plan, then simply cancel the subscription once it has been three months. To cancel a subscription, simply follow this guide: How to Downgrade Subscription Back to Free Account.
