Why description and submit fields are not showing on form?

    Hello

     

    Big bug in form.

     

    Some fields do not appear.

    Fields: Description

    submit button

    Hi UHD31,

    As I find that this is a 2 page form and the "description" and "submit" button were got hidden in the 2nd page. You can check it out by clicking "Suivant" button after feeding in all the required fields (*)
