Where did my form go?

    My account was suspended but reinstated, and now i can not find the form i had to make changes.

    This is because the form was created using a guest account, there are two options, you may clone the form in your account, the one where you're currently logged in, this guide will help you: How-to-clone-an-existing-form-from-a-URL

    We can also move the form from the guest account to your account  jmedina1976, please do confirm us if you would like to proceed with this.

    I have now transferred this form (https://www.jotform.us/form/60107547705150) from your guest account (guest_60107947636056) to the registered one (jmedina1976).
