I get the error "Form not found, this form is disabled"

HiI have created a form but when I view it, I get the error "Form not found, this form is disabled" and it is not disabled on the jotform menubar

I have checked and your account is autosuspened by our phishing system. Your account is suspended because of your form http://www.myjotform.com/form/20655548765565 

I have inspected that form and that form is blank, it does not have any fields. Most likely that form is suspended because of the word "conquer" that is inside of the form title. Considering that form is blank I would suggest you to delete it. 

I have also activated your account, but please delete mentioned form or change title to avoid suspension.
