Hi there, I would be very pleased if someone could help me in controlling forms with a specific field in order to prevent multiple submissions. In my form, from settings I enabled "Name" check. Even though I test the form with different names, it still gives the same error message that this form doesn't allow multiple submissions.

Sure, we can help you with that. You may set form limits based on a unique question/field. I cloned your form to see if the "Name" check you mentioned was implemented. However, it shows you haven't set it up to be a unique question yet in order to prevent multiple submissions.
A better solution is to use a different field rather than the Full Name field as the unique one (e.g. Email Address). Just follow our guide on How to set Form Limits Based on a Unique Question. In fact, this feature is available only for drop down selectors and fields which have a single text box. It will not work for Full Name, Phone or (Postal) Address because they each consist of multiple text boxes.
An alternative way of preventing users from making multiple submissions on a form is to Set Unique Submissions on a Form.

Hi Jim, thanks for your quick response. I tried to follow your advice and set uniqe question as email address. Even though I change email address everytime, I still get a warning and can not submit further.Could you help me on that? Should I change the name of the field from Email Address to Email only?
I checked your form and noticed the following:
I'm assuming you're testing the form submission to see if your changes worked when you said "Even though I change email address everytime, I still get a warning and can not submit further." You don't have to change the name of the field from Email Address to Email only.
I'm seeing a total of 4 different email fields (Email Address, Alternative Email, Email, and Email of the Official). So while testing, please ensure that you'll test with the Email field (the one I highlighted above) on your "Information related to your next of kin" section since that was what you have chosen as your Unique Question. You may change the Unique Question to your preferred field, just ensure you're testing with the selected one.
Kindly give it another try and let us know if you're still not able to submit further so we can have another look.
I am so glad that with your kind help, I sorted it out. Anyone having the same issue should know that Name and Surname fields are not suitable for unique question check in order to avoid multiple submissions. Thank you Jim, thank you Jotform for the spectacular customer service.
