Upon checking your email address email@example.com, I found that you have two accounts with same email address:
Troys - no forms created.
diyplanning - Has bronze subscription plan and one form "Planning Application Report Creator".
I would suggest you to please use the above username to sign in to your desired account. If you do not remember your account password, please user your username to reset your account password by clicking on the "Forgot Password" link which is in "Sign In" window. Please check the screenshot below:
