Trying to log in and it says my account is suspended? Why? Trying to login on jotform.com

Your account is suspended because of your forms:
Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.
Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.
You need to delete credit card fields from all of your forms and only then your account can be activated.
If you want to have payment fields inside of your form you have to use one of JotForm payment tools.

Sorry I didn't realize I still have any forms asking for all the information. You guys contacted me last year regarding this and I changed it and thought I deleted them all. If you can see I haven't used that form that asks for all that info since last year some time. Can you reinstate my account so I can delete that form, please?

I have deleted the guilty fields in your form so we can activate your account. Your account is now active. You can now use your account. Please do avoid having forms that collects credit card information to avoid your account to be suspended again. Regards.

Thank you for activating it. I believe you will have to activate it again since I tried too many times to log in. It now says Autosuspended. I also said to send me instructions for reset of password because it didn't work and I have not received instructions. Can you please send that to me too. Thank you so much! :)

I have reactivated your account. Please delete the forms you no longer need that used to collect credit card information to avoid being auto-suspended again.
Regarding your reset password issue, your email was in our bounce list. That's why you are not receiving email instructions. I now removed it and you should be able to receive the reset password email the next time you try to click Forgot Password.
