    when I receive an email that a form has been completed, it shows the wrong name of the form

    I have never had this problem before.  I used a "membership form" template, but changed the name to "kids ministry form" and when I receive an email notification it says in the subject line:  Membership form.

    How do I get that subject line to change?

    When you modify the title of the form, it does not reflect automatically in the email notifications. You require to manually change the title of the email notification.

    The email notification still indicates MEMBERSHIP APPLICATIONFORM.
