Why heading on my form is incorrect?

I changed the name of the form, but after testing it it comes with the name of the Form I used and hasn't changed to what I typed in. How do I fix that.

I just checked your form and I couldn't find anything wrong with it, the title is the same in My Forms section when I Edit it:

Can you please give us more information when do you experience this issue?

Is your form embedded somewhere? Since if it is you will have to re-embed your form if you did some changes.
