Form isn't working correctly

first it was gray with a frown face...now the text on top of the input labels are not.

Are you saying that the input labels disappeared? If yes, then you can put them back by enabling them in the properties.

Select the field and then click the "gear or options" icon. After that, click "Edit Properties".

In the Properties window, you can click on the "Question" value and enter your desired label. You can also align the label to the top.

I also have different accounts for clients and I can't find their log in.  How can I find out what account this is attached to?  If possible?  Thanks.
