I've provided a URL below, but the problem is happening on all forms I previously created (and which worked fine when originally published).
I have checked your email address and it is in our bounce list, this is the reason why you're not receiving email notifications.
I would suggest you to take a look on this guide in order to set up your email notifications to avoid bouncing: How-to-Setup-Email-Alerts-to-Prevent-Email-Bouncing-Related-Issues

If the issue still persists, please ask your email provider to white-list our IP addresses, this info is on this guide: Whitelisting-JotMails-IP-Addresses
I have also tested your form and noticed that email was sent successfully, you can check the email history as well by following this guide: How-to-View-All-Your-Form-Email-History
