Why is one element in my form missing?

I recently created my first form using JotForm (very cool).  However, I have one field that shows to be present on my Form Manager side - but does not appear in the published version.

Please advise ASAP on how to identify the issue and correct it.  

Time is of the essenvce for this, so please expedite this solution promptly.

I checked your form and it shows the same fields when loading it and when checking it in the Form Builder, here is what I can see: 

Seems like you were able to resolve it, but if this is still there, please let us know the field's name and we will be glad to check your from again. 

I would suggest you to check your conditions, or check if the form is not set as hidden, it may be the reason why it only shows in the Form Builder.
