Please help! When I publish the form there is missing information that is essential!

    Hello!

    When I am looking at my form in edit view I can see the complete information but when I publish it and follow the link there are several missing components. Can you please help me! I need to publish this form for workshops registrations at my nonprofit as soon as possible and I have no clue how to trouble shoot this. I would really appreciate your guidance! Thank you!

    I have just tried to open the URL you provided, but it seems that you have modified the custom URL or have disabled it.

    I'm assuming you are referring to the form:

    Scratch Block Workshops 2016

    When reviewing the form I noticed that you have the following condition. It verifies if there is a video and then hides a group of fields. Since by default there is a video, all the fields selected will be hidden.

    There is also an error in your condition, please verify that the fields selected to be hidden still exist in the form.
