Easy to link Jotform to Mailchimp lists via Zapier, free for up to 100 submissions per month, costs after that. I managed to achieve it by using the Mailchimp Email Beamer private email in a Jotform notification with Gmail with Canned Response and the Auto-responder add-in, free for one responder but only $30 to buy anyway, I think there is a limit on the number of sends for the free version but not sure what it is. Plenty of guidance on how to set it up on the web. In both methods you can filter the fields that are sent out from the form, in Zapier when you set up the link, in the Gmail method when you set up the Jotform Notification email.

Just found the limit for the Gmail Auto-responder, it is 20 per day.
