I checked your account and found out that it was automatically suspended. This automated suspension happens when any of your forms detected by the phishing filter happens to have fields which it may suspect for phishing or any malicious purpose.
I reviewed your forms but did not find anything suspicious. So I went ahead and reactivated your account. You should be able to see your form on your website again.
