I would like to display a table in my form. Is Spread2Jot the best option?

    I want to import an excel spreadsheet to make a form. No submission is necessary or wanted. I don't know if that is the right app for doing it without the submission.

    Anyway, when using the spread2 Jot app, I get a preview of the form that is correct, but when I hit CREATE FORM I don't get what i viewed on preview. See my screenshots.

    It seems to me that the spreadsheet would be very similar to what you are displaying in the screenshot. What happens is that the different options are taken as submissions, this is why text-box fields are created and submissions are imported.

    If you would like to display this information, the best way would be to use the spreadsheet widget:
