Feedback button producing blank forms

Hi,

just created a very basic form and chose to embed the code for a Feedback button.

When I test it on my webpage I can see the button but clicking on it results in a blank form apart from the form title.

The Pop up option works. But the lightbox has the same problem as the Feedback.

Can you please let us know the website URL where you have embedded the feedback form so we can check it here?

But first, we suggest to switch the feedback form to its prototype version by removing the 2 after "feedback" found on the first line of the feedback embed code.

Example,

FROM:

TO:

<script src="https://form.jotform.com/static/feedback.js?3.3.REV" type="text/javascript">
