Form disappeared?

My google form for my wedding RSVP template disappeared and when i click on the link it goes to another persons form saying "houston party" THAT'S NOTMY FORUM and not all of my wedding addresses are lost! How would anyone have access to my forum anyways!!!

Please note that we are NOT related to Google form. JotForm is a form builder that has its own forms. We can only assist JotForm related products and services in this forum page. You might need to contact Google support team for that concern you have, they also have their own forum page here.

I also checked your email address you used on this forum and it is not associated to any of our forms or accounts.
