Why my form is showing as deleted?

Can you please investigate why https://form.jotform.co/unimelb/design-and-print-request-form has been deleted?

I think you have changed the custom link to the form and this is why you're getting this message, I found a form with the same name specified in the link you provided, but the link seems to be different: https://support.jotform.com/unimelb/design-print-form 

I'm not sure if this is your form, but please take a look and let us know, if this is not your form, please share us the form ID so we can find it and know why it has been deleted or why it is showing like that.
