Remove sub headings in address field

I was hoping I would not have to ask this, checked the forum got the answer, I think, then checked back some of my other forms, that work and they matched with the instructions on the forum.

Now I cannot get it to work, it all looks correct, have I missed something?

First of all, you can ask whatever you want. We will be happy to help =) I am not able to see the screenshot that you've sent on my side.  If you want to delete the sub-labels on address field, you can delete them as you see below:

Please, click the sub-label only once, and then delete the sub-label of the address field. If you have any question or issues, please do not hesitate contacting us. We will be glad to assist you.
