My Employee Review Form is not holding format when linked

    I have made an employee review form template. I have saved links and also downloaded pdfs. Neither look like the form that I created. Things are out of order and/or missing completely.

    Unfortunately, I could not fully understand what you're exactly referring to, I have checked your forms and downloaded it as PDF and I can see it is displaying fine, clicked on the print button while loading the web form and noticed that displays fine as well. 

    I'm not sure if this is because of some style missing when you print the web form, but this is becase when the CSS code that styles the form is not supported for the tool that converts the form to PDF.

    Thank you for investigating.

     

    The PDF is wrong. Even the one you made. Under each of the surveys listed there should be a box to fill in comments but they went to the second Page instead and there are only 2

    Name of employee box is missing from the top.

    On the online when you connect to the link. The boxes are in the correct places until you get to the bottom. Then under the Ratings they are missing.

    Also missing the Name of employee box at the top.

    Thank you for providing us additional details about it. 

    I have cloned your form and generated the PDF from my cloned form, it is the same like the PDF form I provided to you. 

    In the Form Builder I cannot find any text box related to "Name of employee", here is an image about it: 

    I can also see there are some fields related to conditions, but these conditions have missing fields, this may be one of the reason why you do not see some fields, I would suggest you to correct this in order to get your form working fine: 

    Regarding to the star rating field, this is appearing like a text box since this field is not supported by the PDF version of forms, this is stated in our guide Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator

    "Form Collapse (5) and Page Break (6) fields are not shown on your PDF jotforms and the Star Rating (1), Scale Rating (2) and Spinner (3) will fall back to the plain input (text box) so you are still able to have the rating entered. The Matrix (4) field works as expected."

    Also, the form is displaying in more than one page because of its length, there is not much that we can do about this, the only way would be reducing the height of some fields, displaying options in radio buttons and check boxes in multiple columns and displaying some fields in two columns as well. 

    To display options in radio buttons and check boxes in multiple columns, you need to click on the field and then the number of columns you want the options to display:

    I don’t mind the two pages. but the missing material was my main thing.
