Not sure why, but I'm not receiving the form info that people fill out when I'm emailed the payment notification. I used to get 2 emails, one for payment and one for the form information...now it's just the payment.
Need advise asap

I would also suggest you to contact you email provider and ask them to Whitelist JotMails IP Addresses.
