    Hi Matt,

I've just opened your form and all the fields seem mandatory. I think the problem might be your browser caching an old version of your form.

Can you try a hard refresh? Assuming you are using Chrome on a Mac, you can do this with Cmd+Shift+R . Or you can use the settings menu (the little wrench right next to your address bar) menu to reach Tools->Clear Browsing Data and tick only 'empty the cache' to clear your browser cache.

Let me know if this doesn't work.

    Hi Matt

Change the URL in the embedded form code from

    http://form.jotformpro.com/jsform/20814595937969

    to

    http://www.jotformpro.com/jsform/20814595937969

Forms at http://form... addresses are served via a CDN (Content Delivery Network) and the node nearest to your location may not be refreshing its content in a timely fashion.

Do let us know if this helps.
