Jotform showing as disabled

Good afternoon, 

We have one of our forms integrated with our ecommerce site, when we go to use the form, it is saying that the form has been deleted (see below) - I went to our forms and was unable to find the form - could you please let us know how we can retrieve the form. I looked in archive and did not find it.
