Terms & Conditions Widget: It is not allowing to submit the form.

I have tested your form and I can see the issue is being generated by the widget " Terms & Conditions ", it shows an error in the web console, I cloned your form and was able to replicate the same issue; however, it should be resolved if you re-add the widget to your form, I did it and worked on my end.

Please if possible, remove the widget from your form and add it once again, if it still does not work, please let us know.
