We noticed that the email and cell number fields were removed from our form without us doing any changes

I do have one more concern that has perplexed us:

We noticed that our form for the 40 & Over State Championship Registration had two fields removed that required players to provide us with their email and cell number. We believe this happened a couple weeks ago when we had an issue with our 18 & Over Registration form as well (also concerning). How could something like that happen? There are only a few of us that can access/edit the forms and we're pretty confident that we didn't make any of the changes.

Sorry for the inconvenience. I have just checked the REVISION HISTORY of the form and noticed that these fields have been removed on May 17.

You may also use this feature to revert back or undo any changes made in your form.
