My form is not working.

    My form is not working. It worked for awhile and now it does not. Can this be fixed ASAP? 

    I took the form down so you can't view it from our website.

    I checked your form and it seems that you have disabled your form.

    You can enable your form by setting the Preferences > General > Status to "Enabled". You can also enable your form in My Forms page. Select the form then click More > Enable.
