Why is the other box looking different on my forms?

On my medical history form I have a few questions with the "Other" box enabled. If someone clicks yes it says "Explain". But when I tried to do the same with my Skin History form it has a checkbox next to the "Other" box. How do I duplicate the "If yes, explain" box with no checkbox?

You are using a  Clean Multi-page theme on your Medical history form.

Just set the same theme on the other form and you should get the same result.
