One of my forms is not editable

In my acc, my jotform called "Balconistas" doesn't work, doesn't appear in dashboard to be edited, simple.

Can you help me? 

I have cloned your JotForm to my account and see that there are no fields in the form. Was your JotForm having any fields? If so, you may restore the form to an earlier version from the form revision history. Please refer to the guide that can help you with this form revision history.
