Thanks for that, its now showing up.
Next problem - Within these 9 fields there is nothing showing up with the reply email, the other fields are filled in, so that's working.

I cloned your form and I can see that you have several fields that are hidden using conditions. Since these conditions cannot be fulfilled, the fields will remain hidden. This is why the first 9 fields cannot be seen in the form. To resolve the issue, please delete all the conditions.
Once they are deleted, the form should look like the image below:
