The form you have linked to, form 61711639983566, belongs to a JotForm account with username unicefethiopia.

This account is on a free plan, and it has gone far over the limits of a free plan. Free plans are limited to having up to 100 MB of uploaded files, while that account has already received 5.11 GB of uploads.

Additionally, free accounts can have up to 500 submissions stored in total at any given time, yet that account has received 15.120 submissions.

The forms will not be re-enabled until the account is no longer over its limits. Considering the number of total submissions you have in that account, and the amount of upload space taken by uploaded files, there are two possible resolutions:

    1. You can upgrade that account to a subscription plan that meets your upload space and submissions storage needs. For example, our Bronze plan allows for unlimited number of total submissions to be stored, and allows up to 10 GB of uploaded files.

Please do note that you must be logged into that account which you want to have upgraded. Only upon logging into your account should you upgrade it.

    2. If you do not want to upgrade the account, you will have to delete a considerable amount of submissions from the account. Only when the number of total submissions and the amount of upload space within that account are within the limits of a free plan, will the forms be activated again.

If you need further clarifications or assistance, please let us know. Thank you.
