Delivery address not appearing in published form

    My form "BRING IT ON - DVD & Download Order Form" has a delivery address field, but this is not appearing in either preview or test forms. ANy ideas why?

    This is because you currently have some corrupt conditions in your form:

    I have cloned your form, replicated the issue and fixed it by deleting those conditions, seems like one of them was affecting the field, then you deleted other associated fields to the conditions and then it generated that the field was hidden without showing since the condition was not working.
