I have sent emails including my PCI DSS Compliance attestation and you are still blocking my forms.  Please respond and UN-block.  You are costing my company valuable time and money.

PLEASE respond to my emails from Toni@ProseCRA.com.

I sent on on June 20th to support@jotform.com with the Compliance attestation and another today.  As of TODAY, you are blocking the information from reaching my office.

    Thank you,

    Toni M. Salopek

Sorry for the inconvenience and delay of the response. I found the email you send to us last week:

I received the attached email from JotForm regarding phishing acitivty (1st Attachment).

Please know that I am PCI compliant.  Attached please find my Confirmation of PCI DSS Compliance Attestation.  This is good until June 2017.

Please let me know if I am good to go AND that either:

    1)    I can put the information back on my credit card form

    2)    If I need to do this myself

    Thank you.

    Sincerely,

    Toni

When reviewing your forms, I found that the form:
